SUMMARY

Responsible to manages the overall process and tracking of all Medicare / Medicaid case-mix documents in order to assure appropriate reimbursement for services provided and coordinates the RAI process assuring timeliness and completeness of the MDS, RAPS and Interdisciplinary Care Plan. The MDS coordinator will also assist the DON with ensuring that documentation in the center meets Federal, State and Certification guidelines.

Essential Job Functions

  • Responsible for the appropriate and timely completion of MDS’s and Care plans, ensuring all supporting clinical documentation contained in the medical records is current, accurate and congruent with MDS.
  • Develops and maintains care plans for each resident in coordination with the resident, resident’s family, staff, physicians, and consultants/vendors (i.e. hospice, dialysis). Coordinates the interdisciplinary care plan team assuring the highest quality plans of care for residents are developed.
  • Responsible for organizing the care plan schedule and inviting the resident and POA (power of attorney) to attend the meetings.
  • Conducts care plan meetings to review care plans with the interdisciplinary team and responsible party and/or resident.
  • Attends morning meeting, and other meetings including QA.
  • Monitors RUG scores assure appropriate reimbursement.
  • Attends all scheduled in-services and meetings. Stays current with Medline University and other continuing education requirements as applicable to the Health Care Center and Assisted Living areas.
